TCM Care Manager – Chapel Hill

Provide assessments, Person Centered Planning, authorization for services, and immediate interventions.

Ensure the provision of services in the most appropriate environment of the consumer's choice.

Conduct screenings, interviews, and assessments professionally, including organizing information for key personnel.

Present assessment information clearly to the treatment team to suggest the level of care for each client assigned.

Complete the required written documentation at the time of service.

Prepare and implement Person Centered Planning/treatment planning for each client assigned

Initial diagnostic orientation with a goal statement.

Explanation and scheduling of services.

Clarification of confidentiality, client rights, consents, and financial requirements.

Describe strengths, preferences, and needs, and identify treatment goals and strategies for each client.

Complete and document the treatment/service plan by the time the client enters the service.

Comply with the goals and objectives of the supervision contract.

Demonstrate willingness to provide clinic coverage as needed.

Maintain a schedule approved by the supervisor.
