What Is Integrated Care?
A person-centered approach to mental health and substance use treatment that connects various aspects of care based on individual client needs.
Methods of Care
Mobile Crisis
On-site support to help calm and stabilize an emotionally charged situation. Mobile Crisis provides immediate, on-site support for individuals experiencing a mental health or substance use crisis. Trained professionals come directly to you to assess the situation, de-escalate, and connect you with the appropriate care. This service is designed to stabilize urgent situations safely and quickly.
Facility-Based Crisis
A medical setting for close monitoring and care during substance withdrawal or medication stabilization, Facility-Based Crisis provides short-term, in-person stabilization in a safe and supportive clinical environment. Individuals receive 24/7 care, monitoring, and therapeutic support during acute mental health or substance use episodes. The goal is to help clients regain stability and transition to ongoing care.
Outpatient Clinics
Outpatient clinics offer flexible and ongoing support, including individual and group therapy, medication management, and peer support services to build long-term recovery skills.
Residential Recovery
Residential treatment offers a six-month, structured, supportive living environment for individuals in substance use recovery. Clients receive daily guidance, community support, and access to therapeutic services to help build stability and independence.
Tailored Care Management
Tailored Care Management provides personalized coordination of services to support each individual’s unique needs. Care managers and peer extenders help connect clients to medical, behavioral health, and community resources while guiding their overall care plan, ensuring a more seamless and effective path to recovery.
