Part-time, Remote – Medical Records Specialist Needed – Temporarily with possibility to become permanent.

  • Ensure the quality of medical records by verifying completeness, accuracy, and proper entry into the electronic record system.
  • Routinely audit records for required elements such as assessments, pcp/treatment plan, service orders, signatures, service notes within the required time limits.
  • Ensure strict compliance with HIPAA, 42 CFR and NC State confidentiality rules.
  • Responsible for ensuring that the Medical Records comply with internal and external standards (Medicaid; Division of MH/DD/SAS; CARF etc).
  • Uploading of documentation in the electronic medical records for consumers in compliance with agency approved protocol.
  • Assist in the collection and data entry for satisfaction surveys.
  • Maintain tracking of archived records by location and ensuring a sign out and sign in system.
  • Conduct ongoing site-based internal quantitative Level 1 audits of charts as specified in policy and procedure to assure compliance with both external and internal standards. Report findings of Medical Records compliance to Quality Improvement Director and Program Managers monthly.
  • Track required information in order to complete consumer’s files. This includes but is not limited to:
    • Documentation and written notification to clinical key members and their supervisor regarding the needed information for files.
  • Keep a log of record review materials and track information as it is competed.
  • Review and follow through with all requests for release of Health Information in a timely manner that is received from both internal and external sources.
  • Maintain a database of request for records released and received.

The ideal candidate will have LCSW, LCSWA, LCMHC, LCMHCA, LCAS, LCASA, LMFT or LMFTA with experience providing both group and individual therapy with adults, adolescents and/or children with behavioral health or SUDs comprehensive clinical assessments crisis managemen. Experience:

Mental health counseling: 2 years

Outpatient Program Director

DESCRIPTIONOversees the safe, effective, and appropriate delivery of clinical and medical services provided through the outpatient clinic and halfway houses.Provide clinical oversight and supervision to the Outpatient Clinic and Halfway Houses at Freedom House, including, but not restricted to clinical assessment, development of treatment plans for clients, client census, aftercare planning, charting, maintenance of statistics and completion of billing paperwork.Provides for the efficient and effective management of the day-to-day operations, both administrative and clinical, as specified in the Scope of Work/Service Definition.Participates in recruitment and retention of qualified staff that enhance the viability of the clinic and houses and further the mission and excellence of the organization. Also works with HR when staff must be terminated.Works with the Clinical Director and Chief Executive Officer to evaluate performance of overall program, including budgeting, census, staffing needs, etc.Ensures that Freedom House policies and procedures are followed in all efforts related to personnel actions.Meets regularly with clinicians, house managers, shift leaders and other staff to provide supervision, ensuring that programs are well managed and meeting their clinical, financial, overall benchmarks.Helps develop and manage program budgets and financial forecasts at least annually.Works with QI/Director to provide quality assurance for all programs including peer reviews, chart audits, CARF preparation, post payment monitoring, and other quality improvement tasks as needed.Acts as agency representative as needed by serving on committees, boards and building community relationships to help maintain standing within community.Works to maintain clinic and group census and to keep beds in halfway houses always filled.Develops opportunities for program growth and development in conjunction with Clinical Director and Chief Executive Officer.

Community Behavioral Health Director

DESCRIPTIONTo provide administrative and clinical management, coordination, and oversight for the behavioral health programs across multiple sites.Program Development/Planning: (a) oversight and review of program priorities; (b) develop and organize internal structures that plan for and can adapt to program growth and meet community needs, and funding source changes.Organizing and Directing: (a) manage employees, assign work, and maintain workload balance for all programs; (b) ensure understanding and adherence to best practices, philosophy of FH and to achieving desired community integration; (c) develop schedules, assign/approve staff to attend meetings, conferences, or caseloads; and (d) continuously assess programs, implement necessary changes to achieve their objectives.Provide continuous assessment of program needs and assist with budget projections to meet needs. Must assure that all program components operate within established budgets.Setting work standards: establish and document programmatic standards or protocol to meet federal, state and local standards as well as standards associated with best practices.Supervision/reviewing performance: (a) perform bi-weekly supervision and review of staff performance, review work against identified program indicators, monitor documentation and medical records, support and encourage staff, review productivity, manage discipline actions, develop performance management plans and work with staff to create short- and long-term individual and program goals and strategies.Quality Improvement/Quality Assurance: establish internal structures and systems which will determine and ensure accountability to local and state standards as well as EBP. Assure and maintain quality services which includes reviewing and analyzing program and consumer outcome data used to make recommendations for improved service delivery. Develop quality assurance measures and regularly monitor and report on their implementation.Performing other functions: (a) assist with development and monitoring of programs’ funding resources and evaluate efficiency of operations and use technology; (b) ensure timely and accurate submission of reports; (c) develop working relationships with other community resources (e.g. law enforcement, local school systems, health care agencies, civic and service clubs, social service agencies, recovery community, etc.).Provides for required documentation of services, forms and group rosters within the time frame required by the FH business office and QI department.
