How to Get Medical Records

Medical records may include information such as visit summaries, medications, and treatment history. You may need a copy for a new provider, continued care, or your personal records. This may take a day or so- we need approval, please do this at your earliest convenience.

On-Line Requests

You can get your medical record electronically by completing this secure form. This is the quickest and most efficient option: we send them to your email within 1 business day.

Mail-in & Drop-off Requests

If you prefer to mail your request or deliver it in person, please download and complete the Authorization Form for Medical Records. Once finished, send or bring the form to your provider’s office by mail, fax, or in person.

In-Person

Please note: bringing the form to one of our locations does not guarantee immediate access to your medical records. Records are not released on the same day the form is submitted. Requests will be processed within 30 days of receiving your signed request.

Third-Party

Attorneys, legal representatives, and other authorized parties may request records by submitting either a signed and dated patient authorization or a valid court order signed by a judge specifying the exact records requested.

Electronic Health Information Exchange (HIE)

We may participate in certain Health Information Exchanges (HIEs) that allow healthcare providers and other entities, such as your health plan or health insurer, to share your health information for treatment, payment, and other purposes permitted by law, including those outlined in this Notice of Privacy Practices. Currently, we are active participants in these HIEs.

Interpreter Services

Freedom House Recovery Center provides a variety of free language assistance services for patients who prefer to communicate in a language other than English. For more information about the available interpreter services, please inquire at Freedom House Recovery Center.
