Tailored Care Management
Tailored Care Management (TCM) focuses on your overall health with personalized plans to help you reach your goals. Our skilled team connects you to essential services like physical and mental health care, food assistance, housing, education, job support, and transportation. Your Care Manager coordinates with you and your providers to ensure you receive the right care when needed, allowing you to focus on your well-being and success.

How it Works
Your local Managed Care Organization (MCO) assigns you to Freedom House Recovery Center for Tailored Care Management (TCM) services. After your assignment, our team will assess your needs. Your care management team will work with you to coordinate services and connect you to community resources. We will have regular check-ins to make sure your needs are met and everything is going well.
Opt-In for Care Management
A member of your care management team will reach out to explain the Tailored Care Management (TCM) program and ask for your consent to support your care needs.
Assessment of Your Overall Health Needs
A meeting will be arranged to evaluate your needs and set your goals. Your care team will discuss your vision for a healthy and fulfilling life, and together, you will create a personalized Care Plan to reach those goals.
Coordination with Community Resources
You and your TCM team will work with service providers and your personal support network to ensure you have access to the services and support you need to achieve your goals, promoting effective teamwork among all providers. If you are prescribed medications, your team will help you access and take them as directed.
Support During a Crisis
Your care team is here to support you throughout your wellness journey. If you are admitted to a hospital or crisis facility, they will collaborate with the facility’s providers to develop an effective discharge plan for your return home and will reassess your needs to update your care plan as necessary.
Ongoing Care for Your Changing Needs
As you progress on your health journey, your needs may change. Your care team will regularly check in to support your progress and update your Care Plan to meet your evolving needs.
Eligibility
To qualify for Tailored Care Management, Medicaid recipients need to have one or more of the following:
- Serious Mental Illness (SMI)
- Substance Use Disorder (SUD)
- Intellectual or Developmental Disability (I/DD)
- Traumatic Brain Injury (TBI)

Partnering with Alliance Health, Trillium Health Resources, and VAYA Health, Freedom House Recovery Center ensures clients receive the right care at the right time from the right providers. Our care teams, who live and work in the communities they serve, are experts in local resources and will collaborate with you and community providers to connect you to essential services, including physical and mental healthcare, employment and educational opportunities, housing, food resources, transportation, and social activities.
Frequently Asked Questions
Who is Eligible for Tailored Care Management?
All individuals enrolled in the Medicaid Tailored Plan can access a Care Manager. If you would like to connect with a Care Manager, please contact your MCO's Member Services Line to request assistance.
Is Tailored Care Management a therapy service?
No, Tailored Care Management is not therapy. It is a service that connects you to and coordinates all the resources you need to achieve your goals, including therapy services, among others.
Will the Care Team come to my home, or do I need to go to them to receive service?
Your care team can visit your home or meet you at one of our outpatient offices to assess your needs and help create a Care Management Plan. After that, they will schedule visits at times and locations that are convenient for you.
Is there a limit to how long I can receive Tailored Care Management?
Tailored Care Management is not time-limited. You can continue to receive the service as long as you remain eligible.
